Initial Treatment & Infertility Testing

Chelsea Fertility NYC is committed to creating an effective and encouraging infertility testing process for both women and men.

The first step on your journey to becoming a parent is infertility testing, which includes a thorough review of your medical history, your family history, and an in-depth physical evaluation using the latest innovations in fertility research. We then perform a series of diagnostic infertility tests for you (and your partner, if applicable) to determine your treatment plan.

When you come in for an appointment for infertility testing, your fertility doctor will offer you a private setting to listen to your concerns, allow you to ask questions, and give you a clear understanding of your options. All of our physicians are committed to serving the emotional and physical needs of patients first. Each infertility condition has its own prognosis and success rates based on the woman’s age and hormone screening, although today these rates are much more clearly defined than they were in the past. Our philosophy is not to exclude patients from the treatments available but to instead allow them to move through options by their own choice. So we will offer recommendations based on your unique situation, but ultimately we want you to feel comfortable with your infertility care and to decide what’s best for you.

At Chelsea Fertility NYC, we have and will continue to treat both good and poor prognostic patients. We choose not to focus on reportable IVF success rates but instead to focus on the needs of our patients, which is in line with our values as physicians. We are physicians, first and foremost.

Pre Fertility Testing

Female:

  • Medical and Reproductive history evaluation
  • Complete physical examination
  • Uterine Cavity Study
  • Sonohystogram or recent HSG (within 2 years)
  • Trial of transfer if IVF is to be done
  • Lab appointment for blood draw on day 1-4 of your menstrual cycle to assess ovarian reserve
  • Including FSH/Estrogen/AMH/transvaginal sonogram for Antral Follicle Count
  • Other Blood tests:
  • Blood Type and Rh factor screen
  • Hepatitis B serology
  • Hepatitis C serology
  • RPR serology
  • HIV serology

Male:

  • Medical and Reproductive history evaluation
  • Semen analysis
  • Semen wash
  • Blood tests:
  • Blood Type and Rh factor screen
  • Hepatitis B serology panel
  • Hepatitis C serology panel
  • RPR serology
  • HIV serology

Genetic testing offered as appropriate (i.e. sickle cell disease, cystic fibrosis, tay sachs disease and others)

Fertility Testing

  • Ovarian Reserve Testing (ORT), also known as Day 3 Testing involves several blood testing and sonograms.
  • Uterine evaluation by Sonohysterogram, also known as Saline Infusion Sonogram, or Hysterosonogram. This may be done in conjunction with HSG or hysterosalpingogram to evaluate tubes as well. Diagnostic office hysteroscopy can be done to give a complete evaluation of the uterine cavity to rule out fibroids or polyps that protrude into the cavity and affect embryo implantation.
  • Transvaginal pelvic sonogram (TVS) for pelvic anatomy assessment and Antral Follicle Count (a form of ORT)
  • Other testing such as endometrial biopsy is still done in select cases to evaluate inflammation / infection and hormone response when indicated such as during a repetitive miscarriage workup or during an evaluation of endometrial response to hormone therapy for mock cycle, frozen embryo replacement.

Pre Fertility Treatment Medication

There are a number of injectable fertility medications. All are given by subcutaneous (under the skin) injection. Many of these drugs have unique syringes or mixing methods. Below is a list of the injectable medications.

  • Follistim ® is a recombinant form of follicle-stimulating hormone (FSH). Treatment with Follistim is intended to stimulate the development of follicles(egg containing cysts) in the ovaries.
  • Bravelle ® is a highly purified form of follicle-stimulating hormone (FSH). Treatment with Bravelle is intended to stimulate the development of follicles(egg containing cysts) in the ovaries.
  • Gonal F ® is a recombinant form of follicle-stimulating hormone (FSH). Treatment with Gonal F is intended to stimulate the development of follicles (egg containing cysts) in the ovaries.
  • Repronex ® is a purified form of follicle-stimulating hormone (FSH) and LH. For women who are felt to need added LH, treatment with Repronex is intended to stimulate the development of follicles (egg containing cysts) in the ovaries.
  • Luveris ® is a recombinant form of LH. Treatment with Luveris is intended to facilitate the development of follicles (egg containing cysts) in the ovaries. Luveris is used in some cases of ovulation induction, controlled ovarian hyperstimulationor IVF in combination with an FSH preparation.
  • hCG is the hormone which is administered to allow for the final maturation of the eggs and to prepare for ovulation.
  • Leuprolide Acetate is used to temporarily decrease your own hormone production and to prevent a “premature LH surge” or ovulation.
  • Antagon® is is a GnRH-antagonist that has the ability to rapidly supress the pituitary glands ability to cause the LH surge. This medication can be used in preparation of an IVF cycle.
  • Cetrotide® is is a GnRH-antagonist that has the ability to rapidly supress the pituitary glands ability to cause the LH surge. This medication can be used in preparation of an IVF cycle.
  • Progesterone is a natural hormone which helps maintain an early pregnancy.
